Fingerprint Card Fill-in Help

Fingerprint Card Instructions:

The information must be Legible and Typed or Printed in BLACK Ink Only.

DO NOT BEND THE FINGERPRINT CARD.

Note: If your fingerprint card lacks your full name, date of birth, place of birth, or Social Security Number, it may be rejected.

Completion of Applicant Fingerprint Card:

1. Applicant’s Name: Last Name, First Name, Middle Name.

2. Aliases (AKA): Any other name you have used (Ex. - a maiden name or previous married name)

3. Citizenship (CTZ): If born in the United States or a US Territory or you are a Naturalized Citizen, print US. If not, print the name of the country you are a citizen of.

4. Date of Birth (DOB): mm/dd/yyyy If unknown, list the approximate age or year of birth.

5. Place of Birth (POB): Include only the state or country using two letter abbreviations. If you don't know the two letter abbreviantion, spell-out the state or country name.

6. Social Security Number (SOC)

7. Residence of Person Fingerprinted: Complete address including, Street, Apt.#, City, State, Zip Code.

8. To conform with the ACIC and NCIC Reporting System, the following Abbreviations are used for physical identification:

Race:

A = Asian/Pacific Islander
B = Black
I = Native American/Alaskan Native
H = Hispanic
W = White
U = Unknown

Height (HGT):

Use feet & inch measurements; Example: 511 (for 5 feet 11 inches, not 71 inches).

Weight (WGT):

Whole numbers only using U.S. pounds; Example: 110 (for 110 pounds).

Eye Color:

BLK = Black
BLU = Blue
BRO = Brown
GRN = Green
HAZ = Hazel
MAR = Maroon
PNK = Pink
XXX = Unknown

Hair Color:

BLK = Black
BLN = Blond
BRO = Brown
GRY = Gray
RED = Red-Auburn
SDY = Sandy
WHI = White
XXX = Unknown
